Post-Operative Care for Bruises and Swelling

General postoperative symptoms that patients experience are bruise, swelling, pain and fever.



Q) What causes bruises?
In the course of surgery, blood vessels get damaged from incision, resection or exfoliation etc, and internal bleeding beneath the skin causes a discoloration of the skin.
A bruise initially appears reddish and it will then turn blue or dark purple and then yellow after a few days.

Q) What causes swelling?
Circulation of body flulid is not smooth on surgical sites, resulting in the build-up of fluid and therefore causing swelling.

To reduce bruises and swelling after surgery, applying cold compression (ice pack) for 2~3 days is recommended since the degree of swelling reaches maximum up to 3 days. And, from the 4th day, applying a warm pack is recommended to help bruises heal.
When applying an ice pack or warm pack on a surgical site, it is advised to place a towel or gauze underneath the pack, rather than placing the pack directly onto the surgical site.

Some of foods are known to be effective for reducing swelling.
- Pumpkin
- Potatoes
- Black beans
- Kiwi, Banana, Grapes, Plum

They help activating blood circulation and inducing metabolism.

Taking a walk or doing light exercises is advised.
And, because having the surgical area elevated higher than the level of heart helps reduce swelling, putting a high pillow underneath the surgical area (mostly a head) is recommended

Making living conditions that can raise body temperature a lot, such as going in a sauna or having an indoor temperature too hot, may delay healing of surgical sites.
And drinking alcohol and smoking should be avoided for a proper healing process after surgery.


Lastly, the best way is to keep the post-operative instructions given by the staff.

[Eyelid Surgery] Ptosis correction (for sleepy eyes)

It is recommended to those who have a weak or defective levator muscle, raising the eyelid, therefore having difficulty in opening eyes widely without moving brows or forehead muscles.

Ptosis can be corrected by tightening the levator muscle.
The surgery is performed concurrently with full or partial incision double eyelid surgery.
If you undergo only double eyelid surgery without ptosis correction even you have symptoms of blepharoptosis, you could get uneven double eyelids or thick creases. Furthermore, eyes might look smaller than before surgery.

Some of anatomic factors may limit the surgical process, therefore the expectations and goals of the surgery must be discussed prior to surgery.

[Facial Contouring] Small or Short Chin

Solutions for Small or Short Chin - Chin implant, Fat graft on chin or Chin advancement


Short chin refers to the case in which the lower portion of a face appears to be absent because the lower jaw bone is situated too far back or the tip of the chin is too small.

In this case, the boundary between the neck and face becomes undefined and thereby giving relative protrusion of the mouth.

Small or short chin can be corrected by different surgical methods: chin implant, fat graft and bone advancement.




When the case is not severe, the size of a chin can be enlarged with inserting an implant (gore-tex or silicone) at the tip of a chin or with fat graft on a chin.

But, for a severe case of short chin, bone advancement is recommended. Chin bone advancement (genioplasty) consists of cutting the chin bone horizontally and moving the lower portion of the cutted bone forward and fix the upper and lower parts with pins.

[Rhinoplasty] Revision Nose Surgery

REVISION RHINOPLASTY


The most frequently consulted case of plastic surgery is rhinoplasty, as many cases as eyelid surgery.

Nose is positioned right in the middle of a face, so it is considered as the main feature of the face which draws the most attention. And, with simple correction or improvement added onto the nose structures, one can achieve more balanced looks.

Because rhinoplsasty includes usage of an implant in most cases, making an imprudent

decision on rhinoplasty may lead to a greater chance of postoperative side effects.
It is found that more than 30% of the patients who had undergone rhinoplasty went for their revision surgery.

According to Dr. Lee Jin Hwa from ITEM clinic, out of many reasons for revision nose surgery, ‘dissatisfying results of surgery’ takes more part than postoperative complications such as infection or contracture.


Many cases where an ‘L-shaped’ silicone implant was used for the primary surgery, without cartilage graft, the dermal tissues of the nose tip may become thinner and result in modification of the overall nose shape. And, as time goes by, the nasal tip may become droopy and the profile of the nose bridge or the tip may change as well.











In case of having a droopy tip or change in the nose shape after surgery, cadaver rib cartilage graft is mostly recommended for revision surgery to strengthen the columella, thus preventing it from modification.

Along with this procedure, ear cartilage graft on the nose tip can result in a balanced height of the tip.

And Dr. Lee advised that patients need to discuss their symptoms and reasons for revision surgery thoroughly with a doctor, specialized in rhinoplasty, who is in charge of a whole process of the surgery.
