The primary surgery is the key for plastic surgery.

Double eyelid surgery has been the most performed plastic surgery as it is the most popular among females and males of both young-aged and middle aged.


But, because it is widely performed and often considered as a 'simple' surgery, people tend to rush on making a decision for undergoing double eyelid surgery.

In many cases of consultation for wide or square jaws, patients bring a question like 'Do I need to get facial contouring surgery?'

If you have wide jaws, you might have developed bones, but it may not be the reason.

[Facial Contouring] Small or Short Chin

Solutions for Small or Short Chin - Chin implant, Fat graft on chin or Chin advancement


Short chin refers to the case in which the lower portion of a face appears to be absent because the lower jaw bone is situated too far back or the tip of the chin is too small.

In this case, the boundary between the neck and face becomes undefined and thereby giving relative protrusion of the mouth.

Small or short chin can be corrected by different surgical methods: chin implant, fat graft and bone advancement.




When the case is not severe, the size of a chin can be enlarged with inserting an implant (gore-tex or silicone) at the tip of a chin or with fat graft on a chin.

[Rhinoplasty] Revision Nose Surgery

REVISION RHINOPLASTY


The most frequently consulted case of plastic surgery is rhinoplasty, as many cases as eyelid surgery.

Nose is positioned right in the middle of a face, so it is considered as the main feature of the face which draws the most attention. And, with simple correction or improvement added onto the nose structures, one can achieve more balanced looks.

Because rhinoplsasty includes usage of an implant in most cases, making an imprudent

decision on rhinoplasty may lead to a greater chance of postoperative side effects.
It is found that more than 30% of the patients who had undergone rhinoplasty went for their revision surgery.

According to Dr. Lee Jin Hwa from ITEM clinic, out of many reasons for revision nose surgery, ‘dissatisfying results of surgery’ takes more part than postoperative complications such as infection or contracture.


Many cases where an ‘L-shaped’ silicone implant was used for the primary surgery, without cartilage graft, the dermal tissues of the nose tip may become thinner and result in modification of the overall nose shape. And, as time goes by, the nasal tip may become droopy and the profile of the nose bridge or the tip may change as well.











In case of having a droopy tip or change in the nose shape after surgery, cadaver rib cartilage graft is mostly recommended for revision surgery to strengthen the columella, thus preventing it from modification.

Along with this procedure, ear cartilage graft on the nose tip can result in a balanced height of the tip.

And Dr. Lee advised that patients need to discuss their symptoms and reasons for revision surgery thoroughly with a doctor, specialized in rhinoplasty, who is in charge of a whole process of the surgery.

Skull reshaping, a solution for flat back of head

According to the International Society of Aesthetic Plastic Surgery, the number of cases of aesthetic plastic surgery in South Korea was ranked 3^rd after the United States and Brazil. As based on this fact, the markets of plastic surgery are reaching 7 billion dollars, and more of various and developed technologies of surgeries have emerged to keep pace with this fever of aesthetic plastic surgery.

Especially, in recent years, skull reshaping for flat or asymmetric head contour has drawn a lot of attention. We interviewed Dr. Chung Woo Jin from ITEM plastic surgery clinic to find out what this surgery is about.

Q) Skull reshaping does not sound familiar.

A) Most of people will question like, “is there such surgery of reshaping back of the head?”

In the past, it referred to an invasive surgery modifying the shape of skull. But, nowadays it is more considered as an aesthetic surgery to make the head contour in a round and more balanced manner.

Q) Who decides to get this surgery?

A) Some may have well shaped and round head since they were born, whereas others have flat or depressed area or asymmetric shape on head from deformation of bone itself or from changes in skull shape due to a habit of lying in one position for a long period of time. Those with concerns about their head shape choose ‘skull reshaping (back of head augmentation)’. 

Skull reshaping using osteobond, which is very effective in contouring the frontal and lateral profile, is recommended to whoever feels they are limited to some hair styles or wearing caps due to their flat back of head.

Q) Please explain the procedures of skull reshaping (back of head augmentation) using osteobond

A) Back of head augmentation using osteobond takes around 30~40 minutes. A 3~4cm-long incision is made on the scalp at top of head, and delicate dissection will be performed around membrane covering the bone to obtain enough space for insertion of osteobond.

Paste-like osteobond is inserted into the desired area, and then it will be solidified into the planned shape. And, scalp closure by using surgical staples and sutures will be performed as the last stage of the surgery.

Surgery is performed under local anesthesia with sedation, and a patient is discharged around an hour from the surgery. Moreover, patients feel less burdened for this surgery because it does not require hair removal for an operation and preoperative period of production of an implant.

Q) Message to people who are interested in skull reshaping

A) The safety of skull reshaping (back of head augmentation) using osteobond has been proven by numerous studies through more than 30 years of observation in postoperative progresses.

Therefore, it is surely an effective method for solving one’s concern on flat back head.

But, it cannot avoid the possibility of side effects such as an infection or fluid collection just same as other general surgeries. So, for safe and satisfying surgery, it is essential to choose a doctor who is specialized and experienced in many cases of skull reshaping and provides the most suitable operative plans to patients.

PRECAUTIONS AFTER THE SURGERY

(FAT GRAFTING SURGERY)

The popularity of fat grafting surgery has been significantly increased due to desiring for baby face. Today, I am going to explain about the precautions after the fat grafting surgery and ways to raise the survival of fat.

(1) AFTER THE SURGERY PROCEDURE 
If you have discomfort due to bandage, compression band, then please do not remove them by yourself. You had better contact with your consultant and please visit clinic to handle them carefully.

(2) SWELLING AND BRUISING

-Swelling and bruising can caused for 48hours and they will be gradually reduced in 2weeks. 

-You have to lie down in 45 degree to prevent from being swelled.
-Applying ice pack can reduce swelling but in case of fat grafting surgery the surgical site is highly probable to be pressed.

(3) DAILY LIFE 
-You can perform your normal life but you should avoid going to hot and humid area such as hot bath and sauna.

(4) WASHING UP&MAKE UP 
-Washing up and make up will be available 5days after from the surgery. But, whenever you wash your face and apply make up, please do not rub too hardly.

(5) SURVIVAL RATE OF FAT 

-To raise the survival rate of fat you have to manage your surgical site for a while.
-Please avoid wearing a hat and sunglasses for about a month to raise the survival rate of fat.

(6) SMOKING&DRINKING 

-You should avoid drinking alcohol and smoking for about a month at least since drinking alcohol and smoking can reduce the survival rate of fat.
