Surgical Method




(1) Angulus mandibulae excision procedure
Long curved mandibular reduction surgery can be operated to make the mandibular angle more smoother. The trimming will be aligned along under the ears of the lower jaw as well as the tip of the chin to be a gentle curvature from the lateral view. Gentle long curved mandibular reduction can change not only the lower jaw into natural and gentle line but also the square facial type into gentle, slim and attractive facial type. This method is most effective if the mandibular angle angulus mandibulae is severely angled and widened and can achieve good results in both the frontal and lateral view. However, in spite of the angled-looking chin, when the mandibular angle is narrow and in particular, inwardly curled at the bottom of the ears, the surgery may give less effects on reduction of the face width, although the quadrangular mandibular angle may be removed. Therefore, in such case, it is effective to perform cortical osteotomy concurrently.




(2) Cortical osteotomy
The bone is consisted of the cortical bone, which is a hard bone that is wrapping around the outside and spongy bone, which is loose and soft like sponge, in the inner aspect. Cortical osteotomy cuts and removes the outer cortical bone that can elevate the slimming frontal effect for the square jaw by reducing the thickness of the jaw bone. In addition, if the cortical bone is cut off, the volume of the muscle can be reduced and achieved frontal view effect which is similar to effect for Botox-Injection.






Consultation and preparation prior to surgery


01. Examination prior to surgery
Surgery plan is established after having analysis of the facial type and accurate head measurement through facial photograph and facial X-ray imaging for accurate diagnosis of the square jaw. Since the square jaw surgery is conducted under general anesthesia, diagnosis is made prior to surgery to determine whether the patient would not have any problem with general anesthesia through thoracic radiography, electrocardiogram, and blood and urine test. So body check-up with blood test should be performed prior to operating a surgery. When it comes to body check-up and blood test should be arranged by ITEM Clinic for patient's accurate diagnosis according surgery procedures.


02. Fasting prior to surgery
Since the mandibular reduction surgery is conducted under general anesthesia, the patient must come to the hospital after having fasted from midnight of the day immediately before the date of surgery.


*After having a surgery, patient will be need of being hospitalization for 1 day. If patient is subjected only to mandibular reduction, it is completed within 1 hour, and in the case of V-line surgery that includes trimming of the tip of the chin, it takes about 1.5 hours. Patients will spend some time in the recovery room until the effect of the anesthesia is fully dissipated and the patient stabilized prior to moving to the inpatient
